Foundayo® (orforglipron) is an FDA-approved GLP-1 receptor agonist used for chronic weight management in adults with obesity, or adults who are overweight with at least one weight-related condition, when used alongside a reduced-calorie diet and increased physical activity. Unlike injectable GLP-1 medications, it is a non-peptide, small-molecule tablet taken once daily by mouth, with no food or liquid restrictions at dosing.

Foundayo® carries a boxed warning for the potential risk of thyroid C-cell tumors and should not be used by individuals with a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN-2). It also carries warnings and precautions for pancreatitis, severe gastrointestinal reactions, acute kidney injury due to volume depletion, hypoglycemia, hypersensitivity, diabetic retinopathy in patients with type 2 diabetes, acute gallbladder disease, and pulmonary aspiration during general anesthesia or deep sedation. It should not be used in combination with another GLP-1 receptor agonist. Patients should carefully review all safety information and discuss any concerns with a licensed healthcare professional before starting treatment.

The most commonly reported side effects include nausea, constipation, diarrhea, vomiting, dyspepsia, abdominal pain, headache, abdominal distension, fatigue, burping, gastroesophageal reflux, gas, and hair loss. These often decrease over time as your body adjusts. Contact your prescriber if any side effects are severe or persistent.

Foundayo® is taken once daily by mouth, at any time of day, with or without food. Treatment typically starts at a low dose of 0.8 mg, increasing to 2.5 mg after at least 30 days, then to 5.5 mg after another 30 days. Depending on response and tolerability, the dose may be increased further to 9 mg, 14.5 mg, or 17.2 mg, each after at least 30 days at the prior dose. Your clinician will determine your specific dosing schedule.

Foundayo® is an FDA-approved medication manufactured under federal quality standards, unlike compounded GLP-1 preparations, which are customized formulations prepared by state-licensed compounding pharmacies and are not FDA-approved. Patients should discuss both FDA-approved and compounded options with a licensed clinician to determine which is most appropriate for their needs.

Coverage for GLP-1 weight management medications varies widely by plan and is often limited. Patients can speak with a licensed provider about self-pay pricing, and some may be able to submit receipts to an HSA or FSA for reimbursement, though coverage is not guaranteed.
